What Is an AI Medical Scribe?

An AI medical scribe is software that turns a patient visit into a draft clinical note. It transcribes the conversation, works out who said what, and organizes the content into a note format such as SOAP, H&P or a consult letter. The clinician then reviews the draft instead of writing it from scratch.

It is different from a human scribe, who is a person who documents in real time or after the visit, and different from dictation, where the clinician speaks the note out loud. An AI scribe works from the natural conversation.

Under the hood there are usually two stages: speech-to-text transcription, then a language model that maps the transcript onto a template. The template decides the sections; the model fills them in from what was said.

What it does not do: it does not diagnose, it does not replace clinical judgment, and it does not make the note final. Generated notes can miss details or misattribute a statement, which is why review is part of the workflow, not an optional extra.

Practical questions to ask about any scribe: Can it use my note format? Does it work with my EHR, or will I copy notes out? What languages does it support? What happens to the audio? What does it cost at my real monthly volume?

MD Record AI is one example: it records live or takes an uploaded audio file, produces a structured draft from a built-in or custom template, and starts with a 7-day free trial. See the AI medical scribe page for what it does and does not include.
