How AI SOAP Notes Work

How AI SOAP Notes Work

Step one is transcription. The visit audio becomes text, with speaker labels so the software knows which statements came from the clinician and which from the patient.

Step two is role identification. Getting clinician and patient right matters because the same sentence means different things depending on who said it: a patient reporting chest pain is Subjective; a clinician stating an exam finding is Objective.

Step three is mapping to the template. For SOAP, the model sorts the content: symptoms and history into Subjective, vitals and exam into Objective, impressions into Assessment, and orders, prescriptions and follow-up into Plan.

Step four is metadata. Title, patient name and specialty can be extracted so the note is easy to file and find.

Step five is you. Read the draft, fix anything wrong or missing, and only then save it. The AI produces a starting point; the clinician owns the record.

See the AI SOAP note generator page for how MD Record AI implements this.
