Ambient Clinical Documentation Explained

Ambient clinical documentation means the software captures the natural conversation of a visit and drafts the note from it. The clinician does not narrate a summary; they just have the visit.

The benefit is attention. When you are not composing the note out loud or typing it, you can look at the patient. The trade-off is that the draft depends on what was actually said in the room, so anything you did or decided silently needs to be added during review.

Consent comes first. Recording a patient conversation is regulated differently across jurisdictions, and organizations often have their own policies. Tell the patient, get consent where required, and document that you did.

Audio quality drives results. A quiet room, a decent microphone and one speaker at a time improve transcription. Overlapping speech, background noise and strong accents make errors more likely.

Review is the safety step. Read the draft for wrong medications, doses, laterality and negations, which are the errors that matter most. The clinician stays responsible for the final note.
